What's the Difference Between Oral Supplements and IV Therapy?

Oral supplements have to survive your gut and your liver. IV therapy skips both, but that doesn't automatically make it the better choice. Dr. Farhan Abdullah breaks down which nutrients actually need an infusion, which work fine by mouth, and how to tell the difference for your own body.

A patient of mine, a 52-year-old commercial real estate guy out of Colleyville, showed up to his follow-up last month carrying a paper grocery bag. Not a metaphor. An actual grocery bag with nineteen supplement bottles rattling around inside it. Three different B vitamins. Two kinds of vitamin C. A liposomal glutathione that cost him ninety dollars. A fish oil that had been open since, by his own estimate, sometime during the Obama administration.

He set it on the exam table and asked the question I get almost weekly now. "Am I wasting my money on all this? Should I just be doing the IV drips instead?"

Fair question, and the honest answer isn't the one either side of that argument wants to hear. Oral supplements aren't useless. IV therapy isn't magic. They're two delivery routes with very different pharmacokinetics, and which one makes sense depends on what you're correcting and how well your gut cooperates.

What Actually Happens When You Swallow a Supplement?

When you swallow a supplement, it has to survive stomach acid, dissolve, get picked up by specific transporters in your small intestine, and then pass through the liver before it reaches your bloodstream. Somewhere between 10% and 90% of the dose makes it, depending on the nutrient and your gut health.

That range is enormous, and it's the whole reason this conversation exists. A capsule of vitamin D3 taken with a fatty meal is absorbed beautifully. A magnesium oxide tablet on an empty stomach is mostly an expensive way to make your morning uncomfortable.

The bottleneck is usually transport. Your intestinal lining doesn't just let nutrients diffuse across freely. Most of them ride specific protein transporters, and those transporters saturate. Vitamin C is the textbook case. The sodium-dependent transporters that pull ascorbate out of your gut max out at fairly modest doses, which is why swallowing three grams doesn't put three grams into your plasma. It puts a fraction of it there and sends the rest to your colon, where it does exactly what you'd expect.

Then there's the liver. Everything absorbed from your intestine drains into the portal vein and takes a first pass through hepatic metabolism before it reaches circulation. For some compounds that's fine. For others it's a meaningful haircut.

And that assumes your gut is working. Plenty of men I see in Southlake are on a daily proton pump inhibitor for reflux, which blunts B12 and magnesium absorption over time. Some have had bariatric surgery. Some drink more than they'd admit on an intake form. In those men, the number on the supplement label and the number in their blood have almost nothing to do with each other.

How Is IV Therapy Different?

IV therapy delivers nutrients straight into a vein, skipping the stomach, the intestinal transporters, and the liver's first pass entirely. Bioavailability is 100% by definition, and plasma levels climb far higher than anything oral dosing can reach. The tradeoff is that those levels also fall faster.

The route matters, and this is the part people get right. When Levine's group at the NIH published their vitamin C pharmacokinetic work in the Annals of Internal Medicine back in 2004, they showed something that reframed the whole discussion: oral vitamin C plasma concentrations plateau around 220 micromoles per liter no matter how much you swallow, while intravenous administration pushes concentrations into the millimolar range, roughly seventy times higher. Same molecule. Same person. Completely different exposure.

So when someone tells you IV is "better absorbed," they're technically correct. It's not that the IV is a superior product. You've just removed every gatekeeper between the syringe and the bloodstream.

That matters most when the gatekeepers are broken. A man with chronic malabsorption isn't going to fix a real deficiency with a bigger pill. And it matters when the therapeutic effect depends on hitting a concentration your gut physically cannot deliver, which is the argument for IV and NAD+ therapy in the first place.

Does Higher Blood Concentration Always Mean Better Results?

No. A higher peak isn't automatically a better outcome. Your kidneys clear excess water-soluble vitamins within hours, so a dramatic spike can translate into an expensive session of very well-nourished urine unless the therapy's benefit actually depends on that peak.

This is where I push back on the marketing in this space. For a lot of nutrients, what your tissues care about is steady-state exposure over weeks, not a two-hour spike once a month. Vitamin D is the clearest example. Your body stores it, your levels move slowly, and the goal is a stable serum 25-hydroxy level in a good range. A weekly IV would be an expensive way to do what a cheap daily capsule does reliably.

Iron is another where the gut is a feature, not a bug. Hepcidin regulates absorption precisely so your body can throttle intake, because free iron is dangerous. We bypass that system deliberately, for specific patients, not casually.

So the real question isn't "which route is stronger." It's whether the clinical target you're chasing is peak-dependent or exposure-dependent. High-dose vitamin C, glutathione, and NAD+ are peak-dependent. Vitamin D, omega-3s, and creatine are exposure-dependent. Getting that backwards is the most common mistake I see.

Which Nutrients Actually Make Sense Through an IV?

IV delivery earns its keep for nutrients that are poorly absorbed orally, that need high peak concentrations to work, or that a damaged gut can't handle. NAD+, glutathione, high-dose vitamin C, magnesium, and B-complex are the main ones, plus fluid replacement itself.

Here's my working list of situations where I'll actually recommend an infusion rather than a bottle:

  • NAD+. Oral nicotinamide riboside and NMN do raise NAD+ over time, but the intracellular kinetics are slow and the data are still messy.   • Glutathione. Swallowed glutathione is largely broken down into its component amino acids before absorption. The intact tripeptide essentially does not survive the trip. I cover the details and the overselling in this piece on IV glutathione and detox claims.
  • High-dose vitamin C. Only relevant if you need pharmacologic concentrations. At nutritional doses, oral is fine and cheaper.
  • Magnesium. Useful IV in specific settings, though most men do perfectly well on oral glycinate at night.
  • B-complex, especially B12. Genuinely valuable in men with pernicious anemia, prior gastric surgery, long-term metformin use, or years on acid suppression.
  • Volume. This one gets mocked, but a liter of saline in a dehydrated man does more for how he feels that afternoon than any vitamin in the bag. In a DFW August, that's not nothing.

Notice what isn't on that list. There's no "immune boost" line item, because that isn't a measurable thing. If a clinic sells you a bag based on a vibe rather than a lab value, you're buying theater.

When Are Oral Supplements the Smarter Choice?

Oral supplements win whenever you need daily consistency, when the nutrient is fat-soluble and stored, or when the deficiency is mild. Vitamin D, omega-3s, creatine, zinc, and magnesium all work fine by mouth in a man with a functioning gut, at a fraction of the cost.

I'd rather a man take vitamin D every morning for a year than get four impressive infusions and nothing in between. Consistency beats intensity in preventive medicine. Not glamorous, but true.

Oral also wins on cost per unit of benefit by a wide margin. If your total supplement budget is two hundred dollars a month, spending all of it on drips while ignoring your protein intake, your sleep, and your vitamin D is a poor trade. What About Liposomal and Sublingual Formulations?

They're a real middle ground, though quality varies wildly between brands. Liposomal encapsulation does improve absorption for some compounds, glutathione and vitamin C among them, and sublingual B12 bypasses intrinsic factor. Neither reaches IV-level concentrations, but for a man who wants better absorption without a weekly appointment, it's a reasonable compromise.

How Do You Decide Which One You Need?

Test before you treat. Get a baseline panel that includes vitamin D, B12, ferritin, magnesium, and a metabolic profile, then match the route to the actual deficit. Guessing based on symptoms is how men end up with a grocery bag full of bottles and no measurable change.

When a man comes in describing exhaustion that sleep doesn't fix, my first move is never a drip. It's labs. Because low energy in a man over 40 is far more often a hormone problem, a sleep apnea problem, or an insulin resistance problem than a nutrient problem. Same with the mental cloudiness a lot of guys describe. Before I'd recommend a NAD+ infusion for brain fog, I want to know his free testosterone, his thyroid, his fasting insulin, and how he's actually sleeping.
